SCOTNURSING LIMITED                                 JOB DESCRIPTION   TITLE: CARE AT HOME WORKER   QUALIFICATION:SVQ /NVQ LEVEL 3 OR SIMILAR QUALIFICATION   EXPERIENCE: PREVIOUS EXPERIENCE OF WORKING IN A HOMECARE SETTING   JOB HOURS:DEPENDANT ON THE NEEDS OF THE CLIENT AND BUSINESS 20+ HOURS PER WEEK   ROLE:TO LOOK AFTER THE PHYSICAL, EMOTIONAL, CULTURAL AND SOCIAL NEEDS OF THE CLIENT USING A PERSON CENTRERED APPROACH   JOB PURPOSE   To support Clients who choose to remain in the comfort and security of their own home to help them live as independent and active lives as possible supporting them using a person centered approach   Responsible for meeting the personal needs of the Client   To observe and promote the Client’s choice, independence, dignity, privacy, fulfilment and other rights   To create and maintain good professional relationships with Clients, their family and friends and other stakeholders   To adhere to all regulatory and statutory obligations and ScotNursing’s Policies and Procedures   JOB RESPONSIBILITIES   To provide complex care and support to a Client with a wide range of needs, illnesses and disabilities   To know and understand the care and support of the Client as detailed in their Care Plan and Client Assessment   To undertake the tasks detailed in their Care Plan and Client Assessment using a person centered approach and in the least intrusive way   To encourage the independence and motivation of the Client   To provide input into the review of the Care Plan and Client Assessment and to feed this back to the Home Care Coordinators in the Call Centre   To assist the Client with personal hygiene including care of the skin, mouth, hair and nails.   To assist Clients in toileting, continence management and personal hygiene. To participate in giving and /or removing toilet utensils and the collection of specimens as appropriate.   To participate in the Moving & Handling of patients in accordance with current guidelines and good practice.   To monitor supplies and resources of the Client   To assist with exercises, physiotherapy and other medical plans as detailed in the Client’s Care Plan   To prepare food/drink for the Client, being aware of Client’s preference , nutritional needs and cultural requirements which are detailed in their Care Plan and Client Assessment   To assist responsibility for the safe handling of Client’s property and equipment   To maintain good communication and develop effective working relationships with the Client & relatives   19.         To provide companionship to the Client, actively listening and talking to them about their interests To accompany the Client on trips/excursions into the community or appointments   21.                      To ensure as safe as possible the Client’s living environment, whilst respecting their choice and rights       RECORDING AND REPORTING   To respect the confidentiality of all aspects of Client care at all times   To maintain detailed and accurate Personal Record of Care Notes and Medication Forms recording tasks undertaken during duration of shift   To regularly read Care Plan and Client Assessment acknowledging any revisions   To report any issues to the Home Care Coordinators in the Call Centre regarding care, support, well being or behavior of the Client and update records accordingly   To continue to monitor where concerns have been reported and recorded   To report any complaints to the Home Care Coordinators in the Call Centre   To attend Client meetings, case conferences and training courses as required   RESPONSIBILITY FOR RESOURCES   To ensure the provision of gloves and paperwork which is sourced from Call Centre ISSUE DATE: 12th August 2016ISSUE No. 1  HND / HNC or equivalent
