An exciting opportunity has arisen for Youth Workers for our well established and busy clients in the taunton area. As a Community Youth Worker you will have the rewarding opportunity to support Young People aged 16 – 21 years, who are leaving care, take the steps towards living independently.   Purpose of post: To work with Young People aged 16 plus, who are working towards Independent living.  To work in partnership with Young People and support them in the transition into independence and adulthood Empower and support Young People to access services and external agencies when and where needed. To aid Young People in Social Inclusion and successfully securing a home/tenancy in their own name To provide a quality service to young people working within government guidelines and The Company’s policies and procedures. Tasks: ·         To Keywork some young people and ensure that the Service Managers are kept updated about them, more precisely about their health, social problems and family concerns or issues, etc. ·         To be responsible for ensuring that the needs of young people are met, that you guide them towards the achievement of positive independent living skills andcommunicate with the Service Managers in order to review how the placements are going/ how the young people are doing and possibly help on monitoring action plans. ·         To be responsible to do a Weekly Report about the young people that you Keywork and communicate with other members of staff in order to give them guidance when working with your young people. ·         To work in partnership with Local Authorities and external agencies i.e. Connexions, CAMHS etc, to ensure the young person’s needs are being met and a high level of service is being delivered. ·         To complete the standards for each YP file and keep updated on a weekly basis ·         Develop good relationships with local communities to promote the inclusion of young people and opportunities for work, training and education. ·         To engage a young person and build a sound working relationship ·         To provide support, guidance and advice to young people in all areas of daily life, this includes budgeting, health, careers, education, relationships, culture, identity, managing accommodation maintenance and safety, amongst many more areas. ·         To encourage independence and self-care skills ·         To work through a Life skills Journal with the person, whilst being sensitive to their needs and abilities ·         To take part in an on call on a rota basis; ·         To undertake sleeps on shift on a weekly basis as and when required; ·         To support the young person in meetings or visits to support agencies i.e. Drugs services, AA and if necessary transport and attend these meetings with the young person. ·         To ensure the young person’s views are taken into account and acted on appropriately. ·         To liaise with the young person’s Social Worker/After Care Worker on a weekly basis.  Providing updates and information regarding your work with the young person ·         To ensure the YP accommodation is ongoing in a good condition and taken necessary action on repairs and issues within the property ·         Ensure utility bills are in the YP name through ensuring they bills are on a meter or card payment scheme. ·         To work with the YP to ensure that on termination of the rental agreement the property is in a satisfactory condition. ·         To complete a weekly report which will be sent to the Local authority, detailing the young person’s progress. ·         Ensure you keep yourself updated with Young Person’s files and information. ·         To attend and contribute to relevant meetings i.e. Pathway plans, reviews. ·         To complete records of visits, phone calls and meetings with young people ·         To prepare and keep up to date files, records and monitoring information.  This includes providing paperwork to Local Authorities and Directors when needed. ·         To complete weekly Health and Safety checks on properties. ·         To adhere to company policies and procedures Do you? Have a passion for supporting at-risk young adults? Have experience in MH or LD? Have an NVQ or equally recognised qualification in Health and Social Care? Drive and have your own transport? Essential information Hourly rate: £8.33 + £35 per sleep Hours of work: 40 hours per week, days, nights and weekends There will be lots of driving involved Must have NVQ II/III, Certificate/Degree/HND Youth Work, Any other qualifications directed to work with Children and Young People Must have DBS on the update service
